Service Description

Service Frequency: We accommodate standing orders with customizable schedules, whether you need daily, weekly, bi-weekly, or monthly blood draws. Our team will work with you to establish a convenient and consistent schedule that meets your healthcare needs. *To book this service, a standing order is required that specifies more than one visit per week or month. * Please have your physician orders/requisition, driver license and insurance card on hand for billing purposes.


Service Policy

StarCare Phlebotomy Services - Service Policy 1. Appointment Scheduling: Appointments must be scheduled at least 24 hours in advance. Same-day appointments may be accommodated based on availability, with an additional fee. Cancellations require at least 12 hours' notice. A cancellation of $25 fee may apply for late cancellations. *DO NOT APPLY TO STANDING ORDERS* 2. Service Area: We provide mobile blood draw services within Central Florida. Requests outside our service area may incur additional $0.67/mile travel fee. 3. Accepted Lab Requisitions: We accept lab requisitions from any licensed laboratory, including Quest Diagnostics, LabCorp, and others. Patients are responsible for providing the correct and complete lab requisition at the time of service. 4. Payment: Payment is due at the time of service. We accept credit/debit cards and other electronic payments. All services include a fee, which covers the mobile blood draw and processing. Patients ordering tests through Ulta Lab Tests are responsible for paying Ulta directly for the tests. 5. Privacy and Confidentiality: All patient information and test results are kept strictly confidential and are handled in accordance with HIPAA regulations. Test results will be shared only with the patient or their designated healthcare provider. 6. Quality Assurance: Our phlebotomists are highly trained and experienced, ensuring safe and accurate blood draws.
