Service Policy

Life Insurance Exam Services Policy 1. Service Overview Scope of Services: StarCare Phlebotomy Services provides comprehensive life insurance exam services, including blood draws, vitals checks, and specialized tests as required by insurance providers. Service Areas: We offer mobile services throughout Central Florida. Please refer to our travel fee policy for locations beyond our standard service area. 2. Appointment Scheduling Booking: Appointments can be scheduled via phone or online booking system. Please provide detailed information about the required services and preferred appointment time. Cancellations: Cancellations must be made at least 24 hours in advance. A cancellation fee of $25 will apply for appointments canceled less than 24 hours before the scheduled time. 3. Fees and Charges: Basic Exam: $75 Services Included: Vitals Check (height, weight, blood pressure, pulse) Standard Blood Draw (general health panel) Urine Collection Brief Medical History Review Moderate Complexity Exam: $160 Services Included: All services in the Basic Exam EKG/ECG Additional Blood Work (cholesterol, liver function, etc.) Spirometry (Lung Function Test) Extended Medical History Additional Charges: Travel Fee: An additional travel fee of $0.67 per mile will apply for locations outside of the Central Florida area. Payment: Fees are payable at the time of service or as outlined in your agreement with the insurance company. 4. Service Delivery Professional Conduct: Our technicians are trained to provide courteous and professional service. Any concerns should be reported to our office immediately. Sample Handling: All samples are handled with the utmost care and transported to the laboratory promptly to ensure accuracy. 5. Confidentiality Privacy: We adhere to strict confidentiality standards in handling personal and medical information. All records are securely stored and only shared with authorized personnel as required by law and insurance agreements. 6.
